All UF HR Documents That Need a Notary
University of Florida’s Human Resources requires notarized signatures on several employee documents. From new hire obligations to benefits enrollment, Stratton Collective Notaries will take care of you. Florida State Employee Loyalty Oath
Required by Florida Statute §876.05 for all public employees, including University of Florida faculty and staff, prior to receiving their first paycheck. The oath must be signed in the presence of a notary public or other authorized officer. New hires who are unable to visit a UF HR office in person can have this document notarized remotely. Failure to complete the oath will delay the start of employment and payroll processing.
Affidavit of Domestic Partnership
Required for UF employees adding a domestic partner to their University benefits, including health insurance and UF Select plans. Both the employee and the domestic partner must sign before a notary. Couples must also be able to demonstrate that they have met the domestic partnership criteria for at least six consecutive months prior to submission.
Affidavit of Termination of Domestic Partnership
Must be filed with UFHR Benefits within 60 days of a domestic partnership ending. The employee must also mail a copy to their former partner by registered mail. This notarized document removes the former domestic partner from all University insurance plans and benefit programs.
Optional Retirement Program (ORP) Retirement Application
UF employees retiring through the Optional Retirement Program (ORP) who submit their forms by fax or email, rather than completing the process online, may need their form notarized. We can assist without requiring you to travel to campus.
Form I-9 Remote Authorized Representative
UF requires all new employees to complete Form I-9 through GatorStart. For employees who cannot appear in person at a UF location, a notary or authorized representative may complete Section 2 on their behalf. Note: Florida notaries may not use their notarial seal.
